More complete informations about this vacancy opportunity kindly read the description below. Performs multiple tasks to get patients prepared for the physician and or scheduled surgery. Assists the physician with medical care to the patient. We will be seeking a nurse practitioner or physician assistant that will be part of a perioperative care team of primary care physicians, specialists, anesthesiologists, hospital-based physicians and surgeons. We recognize that complex surgery is performed with constraints on our contact with patients before, or after surgery. For example, a patient having total joint surgery has ongoing care of their arthritis frequently for years before surgery, and a recovery time of 6 months afterwards. Remarkably these patients spend only 3-4 days in the hospital--it leaves a great deal of care coordination before and after that surgical stay. When we know that two-thirds of these patients have two or more significant medical risk factors, and have a substantial readmission rate, our goal is going to be the redesign of care hand-offs before, during and after surgery. The individual we hire will be instrumental in the development of this work such that we draw into the care platform primary care and surgeons for tightly choreographed care. That said, we would seek a bright individual who has interest in program development, is comfortable rolling up their sleeves to provide direct care but is equally comfortable working with diverse groups of physicians on design of care delivery. We are likely to be participating in a demonstration project on bundled care, a type of accountable care of great interest nationally right now.
Education Requirements: Master's degree; State of South Carolina Certification as a Nurse Practitioner.
Experience Requirements: Prefer 1 to 2 years experience in a primary care clinic or other healthcare setting.
Work Schedule: Full Time 40 hours per week.
